RESUMO

SARS-CoV-2 infection in children is usually asymptomatic/mild. However, some patients may develop critical forms. We aimed to describe characteristics and evaluate the factors associated to in-hospital mortality of patients with critical COVID-19/MIS-C in the Amazonian region. This multicenter prospective cohort included critically ill children (1 mo-18 years old), with confirmed COVID-19/MIS-C admitted to 3 tertiary Pediatric Intensive Care Units (PICU) in the Brazilian Amazon, between April/2020 and May/2023. The main outcome was in-hospital mortality and were evaluated using a multivariable Cox proportional regression. We adjusted the model for pediatric risk of mortality score version IV (PRISMIV) score and age/comorbidity. 266 patients were assessed with 187 in the severe COVID-19 group, 79 included in the MIS-C group. In the severe COVID-19 group 108 (57.8%) were male, median age was 23 months, 95 (50.8%) were up to 2 years of age. Forty-two (22.5%) patients in this group died during follow-up in a median time of 11 days (IQR, 2-28). In the MIS-C group, 56 (70.9%) were male, median age was 23 months and median follow-up was 162 days (range, 3-202). Death occurred in 17 (21.5%) patients with a median death time of 7 (IQR, 4-13) days. The mortality was associated with higher levels of Vasoactive Inotropic-Score (VIS), presence of acute respiratory distress syndrome (ARDS), higher levels of Erythrocyte Sedimentation Rate, (ESR) and thrombocytopenia. Critically ill patients with severe COVID-19 and MIS-C from the Brazilian Amazon showed a high mortality rate, within 12 days of hospitalization.

The aim of this study was to assess the centralization and dentin thickness of mesial root canals of the first mandibular molars by microcomputed tomography (micro-CT). Material and methods: Ninety-nine mandibular molars of Vertucci's type IV canals were scanned by micro-CT. The mesiodistal deviation and centroid were assessed, in both mesiobuccal (MB) and mesiolingual (ML) canals, for the apical 4mm and the full canal length. Results: The dentin thickness was similar for both MB and ML canals. The narrowest thickness was in the distal wall of an MB canal (0.07mm), while the widest was found in the mesial wall of an MB canal (2.46mm). In centroid analysis, both the MB and ML canals exhibited deviations when compared to the root centroid, along the full canal length and the apical 4mm. For the MB canal, the mean deviation was 0.83mm (0.02 mm-2.30 mm) for the full canal and 0.18mm (0.01 mm-1.01 mm) for apical 4mm. Similarly, for the ML canal, the mean deviation measured 0.83 mm (0.05mm-3.99mm) for the full canal and 0.21 mm (0.01mm-1.01mm) for the apical 4 mm. Overall, deviations were observed towards the mesial of the roots, with 69% for MB and 57% for ML canals for the full canal, and 51% for MB canals within the 4 mm. The exception was the ML canal, which exhibited a higher deviation towards distal in the apical 4mm, accounting for 52% of cases. The dentin thickness was consistent between the mesial canals of mandibular molars. However, there is no centrality of mesial canals in their roots, with frequent deviation to mesial.

AIM: To evaluate the influence of different preparation tapers on the reduction in planktonic bacteria and biofilms of Enterococcus faecalis and Candida albicans in the apical third (4 mm) of the mesial roots of mandibular molars, correlating decontamination with canal shape. METHODOLOGY: After microtomography analysis for morphological standardization of the canals, 48 mandibular molar roots, each containing two canals (96 canals), were contaminated with E. faecalis and C. albicans and divided into four groups (n = 11) for canal instrumentation using ProDesign Logic 2 files with different tapers G (.03): # 25.03; G (.04): # 25.04; G (.05): # 25.05; and G (.06): # 25.06 and irrigation with 2.5% sodium hypochlorite. Four roots were examined under a scanning electron microscope (SEM) to qualitatively assess biofilm formation. Eight roots were used as the negative control group (samples were not contaminated). Bacteriological samples were taken exclusively from the apical third of the roots before and after chemical-mechanical preparation and bacterial counts were determined (CFU/mL). The final micro-CT scan was used to quantify the volume variation and unprepared canal area in the apical third. Statistical analysis was performed using the Kruskal-Wallis, Student-Newman-Keuls and Wilcoxon tests for analysis of microbiological data. anova and the Tukey or Games-Howell test were used for analysis of micro-CT data and Spearman's test for correlations (α = 5%). RESULTS: All groups showed a significant reduction in bacteria (p < .05), with no statistically significant difference between groups. There was no significant difference in per cent volume increase between groups. The unprepared area (Δ%) was affected by the file used (p = .026) and was significantly lower for G (.06) compared to G (.03). There was no statistically significant correlation among bacterial reduction, volume and unprepared area (p > .05). CONCLUSION: The different preparation tapers influenced root canal shaping in the apical third but did not improve decontamination in this region.

The objective of the study was to investigate the 5.25% sodium hypochlorite (NaOCl) penetration into the dentinal tubules after different irrigation methods. Seventy canines were stained with 1% crystal violet and divided into groups (n = 20): GEC-EasyClean; GPUI-E1 Irrisonic ultrasonic insert; GXP-XP-Endo finisher; GPC-conventional irrigation and GNC-stained tooth without irrigation. Axial sections (16×) were assessed and irrigant penetration was quantified as a bleaching halo on the surface of the apical, middle and coronal third. In the apical third, GPUI promoted greater NaOCl penetration (p < 0.05). GXP was better than GEC (p < 0.05), as was GPC (p > 0.05). The GPUI and GXP groups were similar in the middle and coronal third (p > 0.05). GPUI and GXP showed better results than GEC (p < 0.05). GPUI was more effective in the apical third and like GXP in the cervical and middle third.

AIM: This study compared intracanal removal of filling as well as the frequency and volume of extruded material after retreatment with either HyFlex or Reciproc instruments in mandibular teeth from cadavers. METHODOLOGY: The root canals of 14 pairs of contralateral single-rooted teeth in mandibles of cadavers were instrumented with Reciproc R40 and filled using lateral compaction. The mandibles were scanned in a micro-computed tomographic (micro-CT) device before and after retreatment procedures. The contralateral teeth were assigned to two groups (n = 14) according to the retreatment protocol using either HyFlex or Reciproc instrument systems. In the HyFlex group, the HyFlex Remover instrument was worked 3 mm short of the working length (WL), followed by HyFlex CM 40.04 and 50.04 at the WL. In the Reciproc group, the R50 instrument was worked up at the coronal two thirds, followed by two more cycles until the WL was reached. Pre- and post-operative micro-CT images were analysed for extrusion and intracanal removal of filling material. RESULTS: After retreatment, extrusion of filling material occurred in 11 (78%) and 14 (100%) teeth from HyFlex and Reciproc groups respectively (p > .05). A similar volume of extruded material was observed after retreatment with both systems (p > .05). A significant decrease in the intracanal filling volume was verified after retreatment with both tested systems (p < .05). However, residual filling material was found in all root canals, regardless of the system. The amount of filling material removed (HyFlex = 80.8%; Reciproc = 65.9%) and the operation time was similar between systems (p > .05). CONCLUSIONS: A high frequency of filling material extrusion was observed after retreatment with the two systems in a cadaver model, with no significant difference between them. Both protocols obtained similar efficacy in filling material removal procedures, although none completely cleaned the canals.

Resumo O objetivo deste trabalho foi avaliar a centralização e a espessura da dentina dos canais radiculares mesiais de primeiros molares inferiores por meio de microtomografia computadorizada (micro-CT). Material e métodos: Noventa e nove molares inferiores com canais tipo IV de Vertucci foram escaneados por micro-TC. O desvio mesiodistal e o centroide foram avaliados para os canais mesiovestibular (MB) e mesiolingual (ML), nos 4mm apicais e em todo o comprimento do canal. Resultados: A espessura da dentina foi semelhante para os canais MB e ML. A espessura mais estreita foi encontrada na parede distal de um canal MB (0,07mm), enquanto a mais larga foi encontrada na parede mesial de um canal MB (2,46mm). Na análise centroide, tanto o canal MB quanto o ML exibiram desvios quando comparados ao centroide da raiz, ao longo de todo o comprimento do canal e nos 4 mm apicais. Para o canal MB, o desvio médio foi de 0,83mm (0,02mm-2,30mm) para canal inteiro e 0,18mm (0,01mm-1,01mm) para o apical de 4mm. Da mesma forma, para o canal ML, o desvio médio mediu 0,83 mm (0,05 mm-3,99 mm) para o canal inteiro e 0,21 mm (0,01 mm-1,01 mm) para os 4 mm apicais. No geral, foram observados desvios em direção mesial das raízes, sendo 69% para canais MB e 57% para canais ML para canal inteiro, e 51% para canais MB dentro dos 4 mm. A exceção foi o canal ML, que apresentou maior desvio para distal nos 4mm apicais, representando 52% dos casos. A espessura da dentina foi consistente entre os canais mesiais dos molares inferiores. Entretanto, não há centralidade dos canais mesiais em suas raízes, com frequente desvio para mesial.

ABSTRACT Root canal morphology and its anatomical variations pose a great challenge to endodontists Aim The aim of this in silico study was to perform a qualitative and quantitative analysis of the threedimensional morphological characteristics of the isthmus in the mesial root canals of mandibular molars using microcomputed tomography (micro-CT) Material and Method Six hundred first mandibular molars were selected, including 317 with two mesial canals with isthmuses between the canals, and fully formed root. Isthmus morphology was determined in 3D longitudinal sections using Fan et al. (2010) classification. Root length, and the volume and area of apical and coronal level were measured. Additionally, the structural model index (SMI) of the canals were also assessed Results The prevalence of isthmuses in the mesial root canals was 32% type II, 29% type III, 22% type IV, and 17% type I. The root length was found to be 9.1±0.5 mm, the volume and area, of all root canal system, were 41.8±40.1 mm3 and 63.6±24.2 mm2 respectively. The isthmi volume and area alone were 11.06±9.03 mm3 and 30.02±11.02 mm2. The study confirmed that isthmuses are present in mesial canals of mandibular first molars, being more frequent in the apical third Conclusion The high prevalence of isthmuses with complex morphological features underscores the importance of using intracanal medications to disinfect areas unprepared by instruments.


RESUMO A morfologia do canal radicular e suas variações anatômicas representam um grande desafio para os endodontistas. O objetivo deste estudo ex vivo foi realizar uma análise qualitativa e quantitativa das características morfológicas tridimensionais do istmo nos canais mesiais de molares inferiores por meio de microtomografia computadorizada (micro-CT) Material e Método Foram selecionados 600 primeiros molares inferiores, incluindo 317 com dois canais mesiais com istmos e raiz totalmente formada. A morfologia do istmo foi determinada em cortes longitudinais 3D usando a classificação Fan et al. (2010). Foram mensurados o comprimento da raiz, o volume e a área apical e coronal e da cavidade pulpar. Adicionalmente, também foram avaliados o structure model index (SMI) dos canais Resultados A prevalência de istmos nos canais mesiais foi de 32% tipo II, 29% tipo III, 22% tipo IV e 17% tipo I. O comprimento da raiz foi de 9,1±0,5 mm, o volume e a área de todo o sistema de canais radiculares foram de 41.8±40.1 mm3 e 63.6±24.2 mm2, respectivamente. O volume e área do istmo isoladamente foram 11.06±9,03 mm3 e 30.02±11.02 mm2. O estudo confirmou que os istmos estão presentes em canais mesiais dos primeiros molares inferiores, sendo mais frequentes no terço apical Conclusão A alta prevalência de istmos com características morfológicas complexas ressalta a importância do uso de medicação intracanal para desinfecção de áreas não tocadas por instrumentos.

ABSTRACT Current instrumentation systems cannot fully prepare oval root canal systems. This may cause accumulation of hard tissue debris and fail to eliminate bacteria from areas inaccessible to instrumentation, which could perpetuate periapical inflammation and jeopardize the success of endodontic treatment Aim To evaluate the performance of two endodontic systems in oval canals by investigating the changes in volume, unprepared areas, and centering ability of XP-endo Shaper (XPS) and WaveOne Gold (WOG) in oval canals using microcomputed tomography (micro-CT) Materials and Method Thirty mandibular canines were scanned before and after preparation with WOG (25/.07 and 35/.06) or XPS (30/.01) to evaluate the volume, surface area, and canal centralization at 4 mm and 10 mm from the apical foramen Results Volume and surface area increased significantly after preparation with both systems (p<0.05). However, no significant difference was observed in the unprepared areas, regarding either the entire canal (26.21% for WOG and 30.10% for XPS), or the apical segment (18.82% for WOG and 14.63% for XPS) (p >0.05) Conclusions Both systems maintained canal centralization, with no difference between them. XPS and WOG had similar shaping abilities in the mandibular canine, but left almost one third of the unprepared areas.


RESUMO Os sistemas de instrumentação atuais são incapazes de preparar completamente os sistemas de canais radiculares do canal oval, o que pode levar ao acúmulo de detritos de tecido duro e manter micro-organismos em áreas inacessíveis à instrumentação. Essas bactérias poderiam perpetuar a inflamação periapical e comprometer o sucesso do tratamento endodôntico Objetivo Para avaliar o comportamento de dois sistemas endodônticos em canais ovais, esse estudo investigou as alterações no volume, áreas não preparadas e capacidade de centralização do XP-endo Shaper (XPS) e do WaveOne Gold (WOG) em canais ovais usando microtomografia computadorizada (micro-CT) Material e métodos Trinta caninos inferiores foram escaneados antes e depois do preparo com WOG (25/.07 e 35/.06) ou XPS (30/.01) para avaliar o volume, a área de superfície e a centralização do canal a 4 mm e 10 mm do forame apical Resultados O volume e a área de superfície aumentaram significativamente após o preparo com ambos os sistemas (p<0,05). No entanto, não foram observadas diferenças significativas nas áreas não preparadas, não apenas em todo o canal (26,21% para WOG e 30,10% para XPS), mas também no segmento apical (18,82% para WOG e 14,63% para XPS) (p >0,05) Conclusão Ambos os sistemas mantiveram a centralização do canal, sem diferenças entre eles. O XPS e o WOG tiveram habilidades de modelagem semelhantes no canino mandibular, mas deixaram quase um terço das áreas do canal sem preparo.

Objetivo: identificar situações adversas no cotidiano de trabalho de profissionais de um serviço de atendimento pré-hospitalar móvel de urgência. Método: estudo qualitativo, fundamentado no referencial teórico de cotidiano de Michel de Certeau, realizado entre julho e outubro de 2020, por meio de entrevistas com 32 profissionais do Serviço de Atendimento Móvel de Urgência (SAMU) de Belo Horizonte, Minas Gerais. O tratamento dos dados foi realizado por análise temática e organizado utilizando-se o software MAXQDA®. Resultados: as situações adversas foram intempéries climáticas; situações de violência; pressão exercida por traficantes, familiares, transeuntes; despreparo dos profissionais e atendimentos em locais inóspitos, de difícil acesso, imprevisíveis, que podem desencadear possíveis incidentes durante os atendimentos aos usuários. Conclusão: a identificação das situações adversas deve fazer parte da rotina antes e durante os atendimentos e a implementação de medidas preventivas reduz os riscos de incidentes ao paciente.


Objective: to identify adverse situations in the daily work of professionals in a mobile emergency pre-hospital care service. Method: qualitative study, based on Michel de Certeau's daily theoretical framework, carried out between July and October 2020, through interviews with 32 professionals from the Mobile Emergency Care Service (SAMU) in Belo Horizonte, Minas Gerais. Data processing was carried out by thematic analysis and organized using the MAXQDA® software. Results: the adverse situations were bad weather; violence´s situations; pressure exerted by drug dealers, family members, passers-by; unpreparedness of professionals and assistance in inhospitable, difficult to access, unpredictable places, which can trigger possible incidents during assistance to users. Conclusions: the identification of adverse situations should be part of the routine before and during the assistences and the implementation of preventive measures reduces the risk of incidents for the patient.


Objetivo: identificar situaciones adversas en el cotidiano del trabajo de los profesionales de un servicio de atención prehospitalaria móvil de emergencia. Método: estudio cualitativo, basado en el marco teórico de cotidiano de Michel de Certeau, realizado entre julio y octubre de 2020, a través de entrevistas con 32 profesionales del Servicio Móvil de Atención de Emergencia (SAMU) en Belo Horizonte, Minas Gerais. El tratamiento de los datos se realizó por análisis temático y se organizó utilizando el software MAXQDA®. Resultados: las situaciones adversas fueron intemperies climáticas, situaciones de violencia; presión ejercida por narcotraficantes, familiares, transeúntes; falta de preparación de los profesionales y atención en lugares inhóspitos, de difícil acceso, impredecibles, que pueden desencadenar posibles incidentes durante la atención a los usuarios. Conclusiones: la identificación de situaciones adversas debe formar parte de la rutina antes y durante las consultas, y la implementación de medidas preventivas le reduce el riesgo de incidentes al paciente.

OBJECTIVES: The study aimed to compare the efficacy of XP-endo Finisher and Passive Ultrasonic Irrigation (PUI) in removing hard tissue debris from curved canals. MATERIALS AND METHODS: Thirty-four mandibular molars with Vertucci's type II mesial canals were scanned in microcomputed tomography before and after preparation with HyFlex EDM, and accumulated hard tissue debris was quantified. Subsequently, the teeth were randomly divided into two groups according to the supplementary procedure: PUI with the Ultra-X insert or XP-endo Finisher. After the intervention, the specimens underwent another scanning. Two separate analyses were conducted, one for the total canal and another for the isthmus area. Unpaired and paired T-tests were used for inter- and intergroup comparisons, with a significance level set at 5%. RESULTS: Both supplementary methods reduced the amount of debris compared to the initial volume. Remarkably, the XP-endo Finisher achieved a significantly higher percentage of debris removal (71% for the total canal and 74% for the isthmus areas) compared to PUI (41% for the total canal and 52% for the isthmus area) (P < 0.05). CONCLUSIONS: Both supplementary approaches reduced the amount of hard tissue debris from canal preparation, still XP-endo Finisher showed a higher reduction compared to PUI (p < 0.05). CLINICAL RELEVANCE: None of the supplementary methods rendered canals completely free of hard tissue debris. However, the supplementary approach with XP-endo Finisher resulted in lower levels of hard tissue debris than PUI in curved canals with isthmuses.

BACKGROUND: Eccentric instruments have been proposed as more effective and less time-consuming for endodontic instrumentation. AIM: To compare biomechanical outcomes of different instrumentation systems and time undertaken for instrumentation in resin prototypes. DESIGN: Sixty standardized prototypes of mandibular second primary molars were instrumented according to the following systems: K-files, ProTaper Next (PTN), XP-endo Shaper (XPS), XP-endo Finisher (XPF), XP-Clean (XPC), and Sequence Baby File (SBF; n = 10/each). Irrigation was performed with saline with simultaneous aspiration, and time spent was recorded. The prototypes were micro-CT-scanned before and after the instrumentation, and image sets were reconstructed and registered. Non-instrumented areas, accumulated debris, removed root material volume, and canal transportation were quantified. Data were analyzed through ANOVA, the Kruskal-Wallis test, and the Wilcoxon signed-rank test (α = 5%). RESULTS: K-files and SBF resulted in more instrumentation time (p < .05). SBF, XPC and PTN removed less root dentine (p < .05), but PTN left more untouched areas (p < .05). Accumulated debris were lower for XPC and SBF (p < .05). Canal transportation was similar among the groups. CONCLUSION: Rotary systems reduced instrumentation time, whereas SBF and XPC resulted in more conservative instrumentation, with less debris accumulation and non-instrumented areas. A dedicated paediatric endodontic system (SBF) outperformed eccentric instruments in terms of effectiveness.

OBJECTIVE: To analyze the daily work of an Emergency Care Unit (ECU), with emphasis on the violence experienced by the multiprofessional healthcare team. METHOD: Case study with a qualitative approach, conducted in na ECU in Minas Gerais, Brazil. The collection took place between August 2020 and January 2021, through observations, interviews and document review. Data were organized in MAXQDA 2020® and subjected to Content Analysis, based on Michel de Certeau's theoretical framework of everyday life. RESULTS: A total of 31 healthcare professionals participated. Violence against professionals was associated with the waiting time, the lack of beds for transfer and the restrictionson the entry of companions. The main aggressions were verbal, followed by physical aggression. FINAL CONSIDERATIONS: The daily life of the ECU was permeated by labor violence. Although it was governed by strategies aimed at organizing the assistance provided, professionals adopted tactics to cope with the adversity.

OBJECTIVE: To evaluate and compare several anatomical parameters of mandibular first premolars from individuals from different Latin American countries using micro-computed tomography. DESIGN: Five hundred extracted mandibular first premolars from Brazilian, Argentinian, Chilean, Colombian, and Ecuadorian individuals were scanned using micro-computed tomography (n = 100 teeth/country). Root canal configurations were classified according to established parameters. Analyses also included: canal volume and surface area, structure model index, distances from the apical foramen to the root apex or the cementoenamel junction, major/minor apical canal diameters, canal orifice shape, and prevalence of ramifications. RESULTS: A single root was the most common anatomy in all countries (range, 97%-100%). Vertucci's type-I canal was the most frequent configuration (range, 36%-66%), followed by C-shaped and type-V canals. The oval-shaped canal orifice was the most predominant in all countries (range, 34%-58%), followed by the circular shape (range, 16%-47%). C-shaped canals occurred in all subpopulations (range, 14%-26%), always associated with radicular grooves. Ranges for canal ramifications were as follows: accessory canals, 36%-73%; lateral canals, 4%-12%; and apical delta, 4%-14% of the teeth. Many anatomic parameters differed significantly between countries (P < .05). CONCLUSIONS: Vertucci's types-I and -V, and C-shaped canals were the most prevalent configurations in the subpopulations investigated. Accessory canals and several complex anatomies were found, with some significantly different frequencies between countries.

OBJECTIVE: This study compared the shaping ability of TRUShape and XP-endo Shaper systems on C-shaped root canals replicas using microcomputed tomography (micro-CT). MATERIAL AND METHODS: Thirty three-dimensional replicas based on a mandibular second molar classified as C1 type I C-shaped canal were randomly divided into two groups (n = 15): TRUShape (G.TRU) and XP-endo Shaper (G.XP) and instrumented with each system according to the manufacturer's instructions. Changes in volume and surface and the unprepared area of the root canal were measured by scanning on micro-CT before and after instrumentation. RESULTS: The unprepared areas were 39% in the G.TRU and 43% in the G.XP group with no significant difference between them (p > 0.05), but both the tested systems left a high percentage of unprepared root canal walls of C-shaped root canals. CONCLUSION: TRUShape and XP-endo Shaper showed a high rate of unprepared areas with similar results after C-shaped root canals replicas for root canal preparation.

OBJECTIVE: The aim of this study was to analyze the internal morphology of deciduous molars through the use of computed microtomography in a sample from Rio de Janeiro. MATERIAL AND METHODS: Thirty maxillary and 30 mandibular deciduous molars (n = 60), divided in first and second primary molars, were scanned by computed microtomography. The teeth were evaluated for root number, root canals, Vertucci classification, root curvature, presence of lateral canals, furcation dentin thickness, structure model index (SMI), volume, and canal surface area. RESULTS: The results showed 100% of maxillary molars had three roots and Vertucci type I canal was more prevalent in this group. In the mandibular ones, type IV was more frequent in the mesial root and class I in the distal root and the cavo-interradicular canal occurred in 2 specimens. Dentin thickness in the furcation region measured 1.53 and 1.59 mm in the maxillary and mandibular, respectively. Volume and area parameters varied according to the evaluated canals and SMI demonstrated that all canals had a cylindrical shape. CONCLUSION: More detailed information about the internal anatomy of the primary molars has been described, which may help strategies in the preparation of these root canals.

Objetivo: Descrever o perfil dos pacientes atendidos pela onda vermelha em 2018 e 2019. Métodos: Trata-se de uma pesquisa quantitativa, de caráter transversal, retrospectiva, descritiva e documental realizada no Hospital de Pronto-Socorro João XXIII, Belo Horizonte, Brasil. O universo deste estudo foram 83 prontuários eletrônicos de pacientes que entraram no protocolo da Onda Vermelha. Foi realizada distribuições de frequência, medidas de tendência central (média e mediana) e de variabilidade (desvio padrão). Resultados: A maioria dos pacientes foi do sexo masculino, com idade entre 1 e 95 anos e média de 33,4 anos. O mecanismo do trauma mais frequente foi contuso, o meio transporte foi ambulância do Serviço de Atendimento Móvel de Urgência, a causa mais frequente dos traumas foi acidente automobilístico seguido de trauma por projétil de arma de fogo. As cirurgias mais frequentes foram laparotomia, toracotomia e craniectomia. Óbito foi o desfecho mais comum. Conclusão: Os pacientes chegam muito graves e o óbito foi o principal desfecho. Sugere-se estudos que possibilitem análise comparativa de dados e padronização do cálculo da probabilidade de sobrevivência. Recomenda-se atualização do protocolo da onda vermelha, incluindo outras cirurgias que já são realizadas e novos critérios de inclusão de pacientes. (AU)


Objective: To describe the profile of patients treated in the "Red Wave", in the period of 2018-2019. Methods: It is a quantitative, cross-sectional, retrospective, descriptive and documentary research. Held at the first-aid post João XXIII Hospital, in Belo Horizonte. The universe of this study was 83 electronic medical records of patients who entered the "Red Wave" protocol. Simple frequency distributions, measures of central tendency (mean and median) and variability (standard deviation) have been performed. Results: Male, aged between 1 and 95 years old, being an average at 33,4 years old. The most frequent trauma mechanism was blunt, the means of transport was an ambulance from the Mobile Emergency Service, the causes of the trauma were an automobile accident, followed by trauma by a firearm projectile. From the performed surgeries, the most frequent ones were laparotomy, thoracotomy and craniectomy. Among the outcome, death was the most common one among patients. Conclusion: Patients arrive very seriously and death was the main outcome. Studies that allow comparative data analysis and standardization of the calculation of survival probability are suggested. It is recommended to update the red wave protocol, including other surgeries that are already performed and new inclusion criteria for patients. (AU)

Resultados: La mayoría de los pacientes eran varones, con edades comprendidas entre 1 y 95 años y media de 33,4 años. El mecanismo de traumatismo más frecuente fue contundente, el medio de transporte fue una ambulancia del Servicio Móvil de Emergencias, la causa más frecuente de traumatismo fue un accidente automovilístico seguido del traumatismo por proyectil de arma de fuego. Las cirugías más frecuentes fueron laparotomía, toracotomía y craniectomía. La muerte fue el resultado más común. Conclusión: Los pacientes llegan muy en serio y la muerte fue el resultado principal. Se sugieren estudios que permitan el análisis de datos comparativos y la estandarización del cálculo de la probabilidad de supervivencia. Se recomienda actualizar el protocolo de onda roja, incluyendo otras cirugías que ya se realizan y nuevos criterios de inclusión de pacientes. (AU)

OBJECTIVE: To assess apical root canal transportation using Mtwo or Reciproc during retreatment of severely curved canals, using a multi-analytical approach. METHODS: Forty mesial canals from mandibular molars (Vertucci type IV) were evaluated before and after endodontic retreatment to compare apical transportation with Mtwo retreatment or Reciproc. The systems were used on the same root but alternating the mesial canals. The analyses were performed by micro-computed tomography. Four evaluations were performed: (a) longitudinal transportation, (b) horizontal transportation, (c) variation in the angle of curvature of the canal, and (d) canal-root width ratio. Horizontal transportation and canal-root width ratio were evaluated in 1-, 3-, and 5-mm levels. RESULTS: Longitudinal transportation occurred in all samples, with a mean of 0.13 mm for Mtwo and 0.17 mm for Reciproc. Horizontal transportation increased as distant from the apex was the evaluated level, in mean. However, the transport was not observed in all samples. The mean angle of curvature was 4.16 for Mtwo and 3.03 for Reciproc. Canal-root width decreased as distant from the apex was the evaluated level. No significant differences were observed considering all tested variables, independently of the retreatment system or evaluated level (p > 0.05). CONCLUSION: Mtwo retreatment and Reciproc presented similar apical transportation (horizontal or vertical). CLINICAL RELEVANCE: Both Mtwo retreatment and Reciproc systems could be considered safe regarding apical transportation.

BACKGROUND: The present study aimed to verify the proportion and factors associated with hospital-acquired malnutrition in the paediatric unit of a tertiary care hospital. METHODS: A retrospective study was carried out in a tertiary care hospital in the state of Maranhão, Brazil. Demographic and clinical data on children and adolescents were collected from medical records and the data regarding weight, height and z-scores of anthropometric indicators were obtained from the World Health Organization (WHO) Anthro® and WHO Anthro Plus® applications. Those with weight-for-height z-score (< 5 years) and a body mass index (BMI)/age z-score (≥ 5 years) < -2 SD at admission were considered to be malnourished. Patients who presented a decrease of > 0.25 SD between the z-score of BMI-for-age (BMIZ) at admission and at discharge were classified as having hospital-acquired malnutrition. Weight loss was also evaluated and was considered significant when it was > 2% between the weight measured at admission and the one before discharge. Logistic regression analysis was performed to verify the factors associated with hospital-acquired malnutrition. RESULTS: The median age was 4.7 years and the length of stay was 21 days; 26.8% of patients had significant weight loss during hospitalisation and a greater proportion had hospital-acquired malnutrition (34.9%). Gastroenteropathies, neuropathies and malnutrition on admission were significantly associated with hospital-acquired malnutrition. CONCLUSIONS: The occurrence of hospital-acquired malnutrition is still a problem in paediatric patients. Thus, providing adequate nutritional support from admission is essential when aiming to avoid deterioration of the nutritional status of paediatric patients during hospitalisation.

To evaluate the internal morphology of 100 distal roots of mandibular first molars using micro-CT. Teeth were scanned to characterise: Vertucci type, root length, canal shape, presence and location of accessory canals, and the number of foramina at 4 mm from the apex, presence of root isthmus and the length from the primary canal to the apical foramen. Vertucci type I was found in 57% of cases, followed by V (27%). The most common cross-section 1 mm from the apex was oval (49%) and circular (38%). The average root length was 16.06 mm (16.61-19.02 mm). The mean foramen size was 0.32 and 0.53 mm for the minor and major diameters, respectively. The volume, surface area and SMI were 7.84 mm3 , 68.87 mm2 and 1.52 mm, respectively. Root isthmi were found in 47% of the samples, and the length mean from the primary canal to the apical foramen was 2,03 mm. The internal morphology of the distal roots of mandibular first molars may be complex and shows variations.

OBJECTIVE: This study compared the shaping ability of the Hyflex CM and XP-endo Shaper rotary file systems in curved mesial canals of mandibular molars using micro-computed tomography. MATERIAL AND METHODS: Seventeen mesial roots of extracted first mandibular molars with two independent mesial canals were scanned before and after root canal preparation with the tested rotatory file systems. Each mesial canal from the same specimen was prepared with one of the two systems. The parameters analyzed were canal centering (transportation) for the cervical, middle, and apical segments, as well as for the entire canal (0-10 mm from the apex); and canal volume increase, canal surface area increase, and unprepared canal walls for two segments, 0 to 4 mm and 0 to 10 mm from the apex. RESULTS: There was no significant difference between both systems regarding canal centering (transportation), volume increase, and unprepared canal walls for the 0 to 10 mm segment (p> 0.05); however, a significant difference was observed for the 0 to 4 mm segment (p <0.01), where the Hyflex CM left 28.46% of unprepared walls and XP-endo Shaper left 13.26%. CONCLUSIONS: The shaping ability of the two tested rotatory file systems in mesial roots of first mandibular molars was similar for all parameters in all the segments evaluated, except for the 0 to 4 mm segment, where XP-endo Shaper left a smaller area of unprepared canal walls than Hyflex CM.
